Reading (23-4) will face third-seeded Chambersburg (19-4) in the championship game Saturday at 12:30 at Giant Center.
The top-seeded Red Knights led just 47-45 with 3:38 to go but pulled away in the final two minutes by hitting 8-of-9 free throws.
Ricki Lopez hit 4-of-5 in the final 1:16.
R.J. Dixon finished with 12 points for Reading.
Tyler Crespo scored 15 points for Township before fouling out in the final minute.
Manheim Township built an 11-point lead midway through the second quarter.
Reading closed the second half on a rush, pulling within 30-26 on Butler’s short jumper with five seconds left.
Dixon kept the Knights close in the first half, scoring 10 points on 4-of-4 shooting.
The Blue Streaks took control late in the first half, using a 9-0 run to take a 17-8 lead. They led 17-10 after the first period.
The Red Knights are in the championship game for the 26th time, looking for their 21st title overall and fifth in 15 years.
They won it in 2016, when Lonnie Walker IV was a junior, but were knocked out in the semifinals last year.
They also won district championships in 2010, 2018, and 2004.
Reading clinched its 43rd trip to the PIAA Tournament with a quarterfinal round victory over Dallastown.
